Form Question & Field Types

The Form Builder offers versatile drag-and-drop components to meet any school requirement:

  • Single-Line Input Field: Ideal for short answers such as First Name, Last Name, or Student ID Number.

  • Paragraph / Multi-Line Text: Large text area for explanations, medical notes, or feedback.

  • Checkboxes: Multi-select options allowing respondents to select one or more items (e.g. sports activities, dietary needs).

  • Radio Buttons: Single-choice selection where only one answer can be chosen.

  • Dropdown Menu: Compact single-selection menu, excellent for grade level or homeroom teacher selection.

  • Digital Signature: Allows parents or guardians to provide a legally verifiable digital signature using their mouse, trackpad, or finger on touchscreens.

  • File Attachment: Enables respondents to upload documents (physicals, proof of residency, or utility bills).

💡 Best-Practice Adoption Tip: Always mark essential identification fields (such as Student Full Name, Grade, and Parent Email) as Required. This ensures that every submitted record can be reliably matched to school SIS records during administrative review.
