Add a Digital Signature Field to a CMS Form

How to include legal electronic signature fields on Edlio website permission slips and consent forms.

Digital signature fields allow parents and guardians to sign permission slips, athletic waivers, and policy acknowledgments directly on your website using a mouse, trackpad, or finger on mobile devices.

How to Add a Signature Field

  1. Open your form in the Form Builder (Forms > Edit Form).

  2. In the left-hand component picker, locate Signature.

  3. Drag the signature box onto your form layout (typically positioned directly after your legal waiver text).

  4. Click the field to configure settings:

    • Field Label: Enter legal confirmation wording (e.g., Parent / Guardian Signature).

    • Required: Check this box so the form cannot be submitted without a valid signature.

  5. Click Save Form.

When parents access the form, an interactive canvas opens prompting them to sign. The signature is captured digitally and rendered directly on all printouts, PDF exports, and admin response views.

💡 Compliance Best Practice: Pair your signature field with a required Date Signed field and a checkbox confirming acknowledgment (e.g. "I acknowledge that my electronic signature has the same legal validity as a handwritten signature") to adhere to standard state electronic records compliance.
